Accurate and thorough questionnaire results will help to ensure timely scheduling and completion of FSS approval.
What to Expect
The Delivery Partner questionnaire has three simple sections: General Company Information, Qualifications, and Personnel. Before switching questionnaire panels, closing your browser or navigating to a different web site, you should click the "Save Data" button to ensure all data for the current panel is saved.

AP - Access Point: a device that allows wireless devices to connect to a wireless network.

MDF/IDF – A cable rack that interconnects and manages the telecommunications wiring between itself and any number of IDFs. Unlike an IDF, which connects internal lines to the MDF, the MDF connects private or public lines coming into a building with the internal network. For example, an enterprise that encompasses a building with several floors may have one centralized MDF on the 1st floor and one IDF on each of the floors that is connected to the MDF.
Circuit Demarcation – The point at which the Service Provider's data circuit ends and connects with the customer's onsite wiring.
- Full – User can access and make changes to any aspect of a network
- Read-Only – User can access network but cannot make any changes
- Monitor-Only – User can only monitor network traffic
CAD – Computer-Aided Design
SSID – The SSID (Service Set Identifier) is also known as the "Wi-Fi Name". The easiest way to find this information is to look at the name of the networks you are connecting to and the passwords you are using. In the worst case, the SSID and Password can usually be found by contacting your network administrator.
Encryption – This information is typically gained from the configuration on your Wifeless APs or your network administrator.

With PoE, switches provide power to APs through Ethernet cabling so that APs will not require an additional power source.

Organization administrators can be set to have full access or read-only access to the organization. Full access will give them privileges to monitor and configure all settings throughout the organization, including adding other administrators (organization and network). It is recommended that organization administrators be selected carefully.
Network administrators can be added as needed by organization admins and other network admins to ahve full privileges, read-only privileges, monitor-only privileges, or they can be added as a guest ambassador. Network admins with full privileges can edit other network admins' privileges and delete network admins. They will also be able to edit network configurations.
A static route is a form of routing that occurs when a router uses a manually-configured routing entry, rather than information from a dynamic routing protocol to forward traffic.

DMS (Dealer Management System) is a company that manages information technology services for other companies via the Web. An MSP client may use internal operations or an ASP (application service provider) to run its business functions.
DSP (demand-side platform) is a system that allows buyers of digital advertising inventory to manage multiple ad exchange and data exchange accounts through one interface.

Dynamic Host Configuration Protocol (DHCP) is a protocol for assigning device IP addresses dynamically on a network. With dynamic addressing, a device may have a different IP address every time it connects to the network.
A Subnet is a mask used to determine what subnet an IP address belongs to. An IP address has two components, the network address and the host address.
NAT (network address translation) refers to a methodology of modifying network address information in Internet Protocol (IP) datagram packet headers while they are in transit across a traffic routing device, for the purpose of remapping one IP address space into another.

Identify the CDK-managed switches at each MDF/IDF. These will generally be Cisco Catalyst 2960s, 2950s, 3560s, or 3550s. These may be labeled with their host name beginning with S1, S2, S3, etc., along with an IP address. Take a clear photograph of each switch.
